• Surviving Relatives (i.e. Sons, Daughters, Sisters, Brothers, Grandchildren, etc.) and where located.
• Spouse (Anniversary Date)
• Church Membership
• Years of Employment or Year of Retirement
• Memberships, Affiliations, Offices Held, Hobbies or Special Interests
• Military Service (include specific branch of Armed Forces)
• Who will be serving as Pall Bearers
• Memorial Contributions (Charity of your choosing)

• Service Information (Clergy, Time, Date, Location)
• Photograph (if desired)
